Active Military
Eligibility Details
Must have a valid ID or current paystub.
Spouses of active duty personnel can sign up with a Department of Defense sponsorship card.
Eligibility:
- Must provide proof of eligibility. Address of primary member name must match that of eligible military member or qualified spouse of active duty military service personnel.
- Customers accepting this offer certify they are permitted to under applicable law.
- Eligibility is subject to annual recertifications for Military discount offers.
Veterans
Eligibility details
Must have a valid form of ID:
- Veterans designated driver’s license.
- Veteran ID Card (VIC)
- VetRewards Card from Veterans Advantage
- Department of Defense Form DD214 indicating honorable discharge.
- Valid retired military ID.
- Current AMVETS membership card.
- Veterans designated driver’s license.
Spouses of deceased veterans must have a Department of Defense sponsorship card.
First Responders
Eligibility details
Must have a valid form of ID showing your name and name of agency. Accepted IDs:
- Employee or volunteer badge.
- Paystub less than 60 days old.
- 1099 tax form from previous year.
- Signed affidavit issued from first responder agency.
Eligible agencies:
- Emergency management agencies
- Emergency medical service (EMS)
- Fire department
- Law enforcement
- Public safety answering point
Flat Rate When You Join
If you do not have insurance or your insurance does not have adequate mental health benefits or you cannot afford the going market rate of $80 - $200 per session then this will provide the flat rate of $60.00/Individual Session or $80.00/Couple Session.
